BMRHC School- Based Health Centers
BMRHC operates five School-Based Health Centers (SBHCs), which provide various healthcare services to elementary, middle, and high school students on or near school campuses. These SBHCs are in the Jasper, Searcy County – Marshall, Nemo Vista-Center Ridge, Quitman, and South Side Bee Branch School Districts. SBHCs partner with local school districts and coordinate with school administration, school nurses, and counselors.
BMRHC’s SBHCs are open to the general community and offer many of the same services as other BMRHC health center locations. They are staffed with experienced Advanced Practice Nurse Practitioners.
School-Based Health Centers (SBHCs) offer convenience for both students and parents. Students can often be assessed, treated, and returned to class without leaving school, while parents typically do not need to take time off work for their sick or injured child. Parents can also join their child’s appointment virtually if they choose.
SBHC Enrollment Packet Information
Boston Mountain Rural Health Center has streamlined the SBHC enrollment process for the 2026-27 school year, particularly for those who completed all enrollment forms for 2025-26.
Page 1 is informational while page 2 contains information that needs to be completed and returned to the school office or the SBHC.
Parents/guardians must complete an ENROLLMENT FORM for each child, whether they are newly enrolling or continuing enrollment in the SBHC.
For those who enrolled children in the SBHC during the 2025-26 school year, no other forms are needed unless BMRHC updates any forms or if you have changes requiring documentation in our system. For new enrollees, BMRHC will contact the parent/guardian regarding completion of other required forms.
Also, new this year, BMRHC can send enrollment forms to you digitally for completion, simplifying the paperwork.
Students must be enrolled to be seen at an SBHC. Below is a link to download a copy of the BMRHC SBHC Enrollment Packet. Please complete and return to your SBHC location.
